Clinical Case Management
Jane Addams Place uses a trauma informed and family systems model to provide comprehensive clinical case management. While the focus is on locating and obtaining independent housing, case management services also include coordination of parenting education, employment, benefits, mental health services, addiction treatment, financial literacy and healthcare.
Jane Addams staff and their partners offer programming on-site that is designed to provide information as well as to contribute to the health of the community. Our programs include parenting, career preparation, financial management, drug and alcohol support, healthy habits, self-advocacy, mindfulness, maternal health and wellness, yoga and art classes and trauma-informed psycho-educational groups.
The Children and Youth team provides on-site programming, links to community activities, and advocacy on the local and national level. Onsite, we offer programming such as homework help, family play group, preschool play, SELF groups, therapeutic arts programming, FIT club, and mentoring. We provide connections to community groups such as Neighborhood Bike Works and Girls Leadership Camp. Staff also participate in the Family Shelter Provider Network Children’s Committee in order to ensure children are receiving all the rights afforded to them under the McKinney Vento Homeless Assistance Act. The Children’s Committee provides literature and trainings to shelters and school district personnel.
Through the Office of Supportive Housing (OSH), staff connect residents to transitional and permanent housing.
Staff link residents to other organizations for access to health care services, drug and alcohol treatment programs, mental health treatment programs, education opportunities, job placement and counseling services.
